You will need the following items in order to re-certify your Pharmacy Technician Certificate. All items must be submitted together in this submission form. Wait until you have all items to move forward.
- Proof of Continuing Education totaling at least 20 hours. These documents must be from a Continuing Education provider. They must include your name, number of hours given, title of course, and date of issue.
- Agenda or list of objectives.
- Payment for re-certification in accordance with the window you choose below.
