You must be registered for a free blood draw or have a copy of your A1C and lipid cholesterol panel for this screening (within last 6 months). If you do not have your labs, please register here for a free blood draw. You will then be able to register for an upcoming heart health screening.

FREE Heart Health Screenings from St. Anthony Summit Medical Center and Summit Cardiology so you can know your risk and take charge of your health.

This Heart Health screening will include a small lifestyle questionnaire, a blood pressure reading, a body mass measurement and the review of your current cholesterol and A-1 c results. If you are not registered for a blood draw or do not have your results from a physician visit, wellness fair or previous 9Health fair, you can register for a free cholesterol/ lipid panel blood draw to obtain these results.

Your confidential information is not shared with any other medical, or business agency including your healthcare insurance provider.
