Record Keeping

As the challenges of delivering high quality dental care increase, so do the requirements of impeccable record-keeping and chart maintenance. Every dental office needs a simple system to ensure that patient treatment and communications with patients are documented in a clear and concise manner. This course is designed to provide you with an opportunity to gain the knowledge and confidence to design and implement an outstanding system of record-keeping in your dental office. During this course, the attendee will be given the opportunity to discuss their real life charting problems and concerns so as to personalize this course to their individual practice situation.

Topics of discussion include:
• Charting systems
• Elements of good charting
• Recommendations about charting with electronic media
• Documenting dental procedures
• Documenting unusual occurrences and poor outcomes
• Guidelines for adequate charting
• Documenting office communications with patients
• Informed consent

After attending this course the attendee will
• Be able to implement a simple, yet state-of-the-art approach to record-keeping in the dental office
• Gain insights in how to chart and document complex and difficult situations and circumstances that inevitably appear in busy dental practices.


Dr. Stephen J. Persichetti traveled to Portland after graduation from the Baltimore College of Dental Surgery at the University of Maryland. The purpose of his visit to Portland was to receive advanced training in general dentistry in a residency program at the Oregon Health Sciences University. After completing his year of residency, Dr. Stephen Persichetti joined the faculty at the Oregon Health Sciences University School of Dentistry. He has been an associate professor at the dental school since 1985. He joined Downtown Dental Associates in 1997. 

Dr. Stephen J Persichetti is a Fellow in the Academy of General Dentistry, the American College of Dentists, and the International College of Dentists. He is past president of the Oregon Academy of General Dentistry and served on many of the Academy of General Dentistry’s councils. He is affiliated with Donated Dental Services and treats many patients who could not otherwise afford dental care.
