During this session, Transition Team Leaders will gain knowledge and skills that will support them in implementing best practices in postsecondary planning for students with disabilities. They will do so by identifying the postsecondary pathways that students may pursue, aligning their college and career readiness initiatives to increase postsecondary preparedness and building awareness around available resources.
